    Take the First Step:

The first step to becoming a patient is scheduling a telehealth visit with our patient navigator. Call the clinic to schedule: 720-848-2070. If you are unable to complete a telehealth visit, we will schedule you an appointment in person.

In this intake visit, you will learn about the Surgical Weight Loss Center’s program requirements. With the patient navigator, you will cover pre-op journey, insurance requirements, and general surgery timeline.

There will be an intake questionnaire sent to you through My Health Connection when the visit is scheduled. Thank you for taking the time to fill out the questionnaire prior to your visit.

We look forward to meeting you and partnering with you in your good health!
