I'm sorry she has to go through this Cricket.  Does she have any other doctor that could sign off for her?  I filled the app out myself and my doctor signed it after looking it over for a couple days.  Maybe and OB or some other specialist that would do it for her?  Good luck to her!

I know that is what we are thinking I think she has a huge amount of ammunition to go back to him and give him the information.. Thanks to everyone for their input...He was previously a Dr in the US and said he had been screwed by the American Medical Assoc.. some things just make you go Hmmmmmmmmmmmmmmm..and that would be one of those things.
