Unbearable pain (TMI possible hemmeroids/fissures/poop issues)

I posted this on the FB Group but I know not everyone is on it so I am posting it here too.

I had to go to the ER this AM as I was in so much pain. My ass is killing me and hurts so bad. I was so embarrassed, but couldnt take the pain anymore and my PCP couldnt fit me in. I have been having on and off pain for a few weeks now, usually it would go away and I thought it was just sore from wiping so much. About a week ago I called my PCP and he told me to pick up some hemmroid cream to see if that helped, well it didnt. Then with my upped dosage of Iron my poop has been firm instead of soft and watery like it usually is and its hurt so bad to go to the bathroom.

I can feel a little spot near the opening which I think is the hemmroid he was taking about. I know he stretched me out pretty far while looking but I would have thought he would have looked with an instrument not just his eyes and his fingers which literally about killed me. I also had some blood streaked poop (I know I am sorry TMI) but then I also got my period yesterday. I am also bleeding a little bit when I wipe and feel raw down there.

So the ER Dr was pretty useless. He stuck his finger up my tush which about sent me through the roof. He said he saw one small hemmroid but didnt know if that was the issue and it could be fissures but he didnt see any. I get dressed and hesays wait let me check and make sure there isnt a mass so I get undressed again and he sticks his finger farther in and about kills me. I am crying at this point and before then I couldnt sit because it felt like fire ants were all over me in my tush and around it. So he gives me 5 pain pills and tells me to use a cream (which I already am) and told me to get a stool softener that I need to get my poop back to where it was and go more often. The one he recc however is a gel cap so I am not sure what to buy for a stool softener and if I need a laxative or not? I am going to the bathroom its just firm and killing me. He also said to take baths with Epson salt a few times a day. I see my PCP this week so I will be following up with him but right now I am in so much pain its unbearable. What is there for me to do?

Awww...I'm so sorry.  In the future, to avoid the ER, you might try looking for a minor emergency/urgent care clinic.  They are gaining in popularity and always take new patients and lots of different kinds of insurance.

I don't have much to provide except hugs, well-wishes, and a recommendation on the stool softener: colace.  Try at least 3 capsules to start.  Keep using daily to avoid hardness.

You can also try miralax.  Unfortunately, there's not a WHOLE lot you can do about the poop already in there.  You can try a glycerin suppository, which draws liquid into the rectum, and that'll most likely make you poop within 15 minutes...but it might not make it as soft as you'd like.

Kayla: Thanks so much. This was actually the old urgent care and they turned it into an ER but its not what the normal ER is like and you almost never have to wait more than a few minutes and they are only open certain hours. Weird I know, they added more stuff and charge a ton more! I went there because I knew the other ER that is the real ER would be packed as usual and it takes hours to get in and out of there.

I cant remember what the one I got is called and it hurts too much to keep getting up but it has a laxative and a stool softner in one. It was the only one I could find that was not a gel cap stool softner.

After the Dr's fingers in their I dont know if I can do a suppository. It hurts so terribly bad I cant even begin to describe. He didnt even warn me either and I thought a small camera was going to go in there and look around if anything not a big finger. It was awful!

I am still going poop about 2-3x a day which is less than usual but the problem is its hard and it hurts really bad. I dont know if that means if I need the laxative too but since thats the only thing that had a stool softner that wasnt a gel cap I got it. He told me to use a laxative plus milk of mag plus a stool softner plus gel/cream and do 3-4 salt baths a day. He also told me to follow up with my PCP who I see on Wed and then possibly with a general surgeon as he told me they are the ones who deal with this. Thanks again!

Gail: Hugs! I am sorry you had to go through this too. Does it come and go or it just went away and stayed away? I have tried all the OTC wipes, creams, gels etc and they all make it feel like my tush has fire ants all over and in it. I have no idea why but obviously that makes it hurt worse.

Did you have that issue? I dont know if its just so irritated down there that it doesnt matter what I put its going to be painful or if its something in the wipes and creams.

I actually have been going through this also. I know exactly what you are talking about with ass pain. Mine was so painful at one point I felt like puking while I was pooping.

My doctor prescribed a cream for the hemmroids but that didn't really help. I have my own supply store of desitin, prep H, prep H wipes, etc... I started to research it on my own and one article said to not eat any nuts, nachos, fritos, spicy or saucy food, acid (orange juice) etc.. also don't use the wet-wipes on your fanny because they can be drying (the scent in them, even unscented has some suds). So after I go I actually take a cold paper towel folded and hold it down there to stop the throbbing and then I dry and put Desitin maximum pain on it.

It took a long while (over a month) and there are still some days but it is better than it was before. Believe me, I feel for you.

Lisa: I am too sorry you had to go through this. So its still off on and on? Do you take things all the time to prevent/help? What did your Dr give you?

I havent been using any wet wipes and out of that food group have only had a few peanuts but that was the other day. I thought I was the only one with that throbbing. I have been putting witch hazel on a cotton ball and doing that and it feels better for a second but then starts burning again.

I never in a million years could believe this causes so much pain.
Its also really crappy that they dont know if its hemmorids and fissures or only one or the other. They said it would be treated the sme way but still would be nice to know what is going on for sure. Hugs!

Yes it is still on and off but much better since I have been following the diet. My doctor said mine was hemmroids and he prescribed protonix. I don't think it really helped. I was also doing the witch hazel and I thought it helped but I think it was drying also.

When I started the diet and used a cold wet paper towel (just fold it up and hold it down there.... what a relief) and desitin that is when things started to change around. It was literally over a month though. I was in pain for hours after having a BM and it felt like my heartbeat was in my ass (sorry for being crude). It throbbed for hours and got better as the day went on. I also soak in a warm bath twice a day.

I wouldn't wish this on my worst enemy.... horrible... I also had thought about going to the ER but figured they wouldn't do much for me so I waited until I saw my gyno for my yearly.

Hi and sorry you are experiencing this. One thing you may want to ask you PCP about is a rectal anesthetic. It may help with the pain. HTH

This link may help you in the future once you get this immediate issue resolved.

My plan to keep from getting constipated:  400 mgs of Mag Oxide with EVERY calcium dose.  Popcorn w/ 1/2 stick of butter 5 nights a week.

Sorry you are in so much pain.  Doesn't sound like the ER dude was a bit of help.
