Federal BCBS

Does anyone know what the comorbidities gastric bypass BCBS Federal has?

Call the number for customer service on the back of your insurance card or go to their web site.
I have regular BCBS (not federal) and I was able to get the actual "printout" of the policy guidelines for WLS - it was so helpful. 

I have Federal BCBS, I had a BMI of 39 and was diabetic.  I have no problem getting my surgery.  From reading my brochure it looks like the requirements are the same now.

 hypertension, diabetic, sleep apnea, gerd are a few....BCBS approved with with light sleep apnea..(no machine), gerd and PCOS
