Good question for the DS forum. I have heard that a revision from sleeve to DS does not cause as much weight loss as getting a DS in the first place. But either revision will give you malabsorption and weight loss. Long term malabsorption is with the DS, so that should be most loss and easiest to maintain. This should make for interesting research for you. Good luck.

I've seen some sleeve to DS revisions lose a LOT of weight! The good thing about the DS is it helps to keep weight off more than the other surgeries. To be honest though, I have seen a few sleeve to DS revisions that didn't lose much weight. However, I bet they will not experience as much regain with the DS. The only people I've seen not lose a lot of weight with the DS or gain a lot back are people who eat a lot of simple sugars and carbs like ice cream, cookies, donuts, candy and stuff like that.
No one can predict exactly how much weight any individual patient will lose with either DS or RNY. However, statistics tell us that the DS has the highest percentage excess weight loss of any bariatric surgery presently available. It also has the best track record for weight loss maintanance, AND for resolution of type 2 diabetes. With RNY, about 84% of patients appear to have their type 2 diabetes resolve at first, but for many, it returns after a few years. With the DS, rate of resolution for type 2 dm is 95 - 98% depending on which study you read, and it rarely returns.
There is more for you to consider than just weight loss with this decision.
