HELP!!! I have to appeal for my sleeve

I have done searches and found information, even a person who shared her appeal letter, but everything is from 2008 and back. Can anyone point me in a direction to get some current help with an appeal? I don't even know where to begin.

How about some more information -- like why were you denied??

Because it is not a covered procedure.

Unfortunately, unless you have an extreme situation you're kinda screwed.
