Is smart forme out of business?

Its on the list of approved shakes but I cannot find their website.  

I tried looking too but couldn't find anything current and I've not heard of them myself. There are lots of other shakes out there I would try some of them or better yet try getting some samples from some of the companies. I know Walmart and Costco carry some in case you need something right away. Good luck!

I looked also as the other poster did and could not find anything. I have a suggestion that you contact your doctor's office and ask if there is an alternative for you to use instead.
