Your skin accounts for approximately 12% of your body weight.  At 220 lbs, your skin took up 26.5 lbs of your total body weight.  It is impossible for you to have 20 lbs. of excess skin.  You likely have 8 lbs of excess skin -  (12% of 220 lbs - 12% of 150 lbs).

Consider this...
Human skin weighs a maximum of 15% of your total body weight (most experts say between 10-12%) .

You started at 245lbs so your skin weighed 36.75 pounds.
Your current weight is 135
So your skin should now weigh 20.25 lbs,
that means the most you could have 16.5 lbs
assuming your skin has zero elasticity

but using the experts numbers you started with 29.4 lbs of skin
and at your current weight should have 16.2 lbs
so you have an extra 13.2lbs skin weight.
Also assuming your skin has zero elasticity, which is unlikely.

There's no way in hell you have that much excess skin. You really should get a 2nd opinion. I lost 200 lbs of excess weight and had 17 lbs of excess skin removed. You know what has to be done with excess skin so if you can't afford PS then invest in some good undergarments and learn to live with it like so many others have to.
