Food journal for dietician/nutrition appointment...
I find your word choice of "good journal" interesting.
The Ottawa Weight Management Clinic provided us with forms to use for one week prior to the Dietician appointment.
I journaled all my food intake for that week, good, bad, indifferent. If you don't journal what you normally eat, in my opinion, you're setting yourself up.
Since you weren't provided with a template, might I suggest journally a ten day period, using, for example, My Fitness Pal? You can then print off each day and take them with you.

Hi Sassy! I did one week without any changes in eating habits chips and everything were written in. Then I did about 102 weeks showing the changes I was making. That way they could see where I started from and where I was headed. I used myfitnesspal.com to keep track and printed them out before I left. Good luck!
Thanks everyone for your great replies ;) I am going to start on Monday. Hopefully this will be good. I have tried my fitness pal but I'm not that good navigating it.
Chipmunk, no worries :) I would have probably questioned that too!! Lol when I read your reply I didn't understand until I went back and re-read what I wrote!! Haha
