How to add surgeons name

I have been trying for quite sometime and very annoyed. Can someone please chime in to let me know how to add my surgeons name to my profile,eventhough I don''t have a surgery date just yet. Every time I try it wont allow because it's says I'm leaving out the field where the surgery date goes.....ughI see that other OH members have a surgeons name w/out a date, so It can be done.
