dr g put the payment fee requirement in place after your surgery, and they have decided no waivers, even though i was promise before hand. If he doesnt accept the tricare offer. i guess i will be going to lenox hill.

Topic: RE: I have a consult date with Dr. Gagner!
You can take amtrack into nypenn, and a cab the 4miles to the hospital, or if you drive in the best parking is on 68th under the hospital,u get off the fdr at 71st, up to the light, south to the next corner and turn in, max for the day is 24bucks, just leave the super suv at home they surcharge them. I would suggest get a appointment with liz goldenburg for the same day. she is the nutricionist. I saw dr dakin his associate on a thursday because he was in net,and he refered me to gagner, as he was a rny,not ds doctor, so i am doctors dakins patient and dr gagner is "assisting' on the ds. send me a email and i will tell you more about how that is working out. i did get full reimbursement for dr g and liz, from aetna.

I expect Aetna to be approveing my ds, they requested i go to a in net provider for consult in stead of the in net rny spurgion, who submitted for me requestting a out of net assist for the ds. next if your going to self pay, set up a health care reimbursement account, makes the whole thing tax deductable. dollar 1, and when you self pay you will be paying retail, instead of wholsale prices, that Aetna will be reimbursing, you will be beat even if you get them to reimburse. dont give up yet.

I take my vw to get fixed and it cost 75 dollars a hours, take a porcha to the same dealer and it cost 120.and hour to change a tire. Maybe if more doctors undertake doing the ds, it will be more available and the cost will be competative. Unless you have some particular needs perhaps such as a revision to ds from rny, or a wierd one like me , a ds under a nissan wrap. where the doctor is going to adlib his way thru it. you should be fine.

I'm more optimistic, than my co-respondents. She is a licensened bariatric surgeon, Is it significantly different to do a vertical cut then a horizontal cut, and poke a hole in the pouch, and attach a piece of intestine,is the sewing of intestines in a ds configuration different than sewing it in a rny, meaning ,sewing the intestine is still sewing the intestine. the routing is different, but that should be able to be handled. I am assuming she has done a few rny's, she is not developing her own methods , she is following a cookbook, and has the surgical skills already proven.

You got my attention, monitoring closely. I had a recent talk with the people in the appeals department about my" not in appeal yet folder under review by the union demand" because i wanted to have them define the vague rules of cpt 157, and i ended up coaching them and having them go ask my questions of others ,ended up in a conference call, not a confrontation but clearly expressed my intent to comply with what ever they dictate, but do it now clearly, see another doctor, got the ref for monday, see a behavior modifiyer got a referal monday, excersise, got a consult with a orhorpedic to cancel that one. made them lay it out and i promised to spend thier money and take time off from verizon to thier specs, no problem.

Topic: RE: Got GERD?
got barrettes, the ultimate cure for gerd is a nissan fundplacation, which precludes haveing a rny, but the fundus is removed in the ds, so the ds precudes the wrap, i will have my ds under my wrap, sort of 1/3 stomach removed instead of half. still effective because the fundus, is wrapped around the esophagas to cause a spicter. Your surgeon may want to consider this in planning your surgery.

Topic: RE: Weird carb question?
I suppose that the carbs injested are attached to more inert molecules than the protiens, so the output of the inert material will be greater, but the solutable digested carbs that are converted to glucose go to the blood stream. you loose the theory and gain the pounds.

My approach would be to get the pump if indicated and insurance buys, the thing about diabetes is the silent damage done now, nueropthy doent go away, and the damage done now will be come evident much later. so treat the diabeates agressively now, and all going well in a month throw the pump away, but do damage now by delaying treatment is not a GOOD thing. im going to check my sugar now.

You can to go now to the public defenders office, get a leg up on the situation, and arrange for surrender immeadeately before the arraignment and perhaps miss the whole arrest proceedure, and confinement, not aware of any particulars except this post, but if a promise of arrest it cant be all that bad, it can be managed.

Topic: RE: rny to ds
You should be good, although tricare does not do the ds, they might allow me, cant have the rny, appeal in progress, medicare does which tricare for life is secondary to,so your medicare deductible should be covered. confirm with tricare pre approval first.

dont forget that the 2500 can be total tax deductable with a health care reimbursement account, but you have to plan now for next year.

Topic: RE: Dental work before DS?
I would suggest getting it done asap, and the cleaning first, lack of dental care can be a root cause of infection , periodontics should be in order before any surgery,

Topic: RE: Blood Pressure woes!!!!!!!!!!!
Had an incident on tuesday morning when after retireing for about 1hr i awoke with a very rapid heart rate,170 with a bp of 160 over 90,normal for me is bp 68 and 108/70, palpatations running wild, I was in tacardia afib, blood sugar was 97, like an idiot,i self dianosed and chilled and luckily did not die. I am a emt, know much better. the cardiologist didnot mince words when i checked in the morning. anyway,he had done a nuclear stress test two weeks prior, so i was mistakenly comforted by that. all my pre certs for surgery are done. saw my pcp later for a ekg and exam in reference to the event, she expressed concern that i may be suffering from my sleep apnea, (purpose of my story for you) she is also my pulmanist. I cannot tolarate the full rx that i require for my apnea, and can only work with about 80% of the pressure in my bipap. my bmi is 38, you indicated that you gave up on your cpap, have you had a sleep study to confirm discontinuing the cpap, you may need o2 tharapy durring your more strenguous therapy sessions, a o2 proximeter worn during exercise will dianose that in a heart beat. until you loose some more, low blood o2 will cause the heart to race, vessles to contrict and bp to rise to get more blood to the brain to compensate. just a laymans imput.

Topic: RE: GERD and DS?
the ds is a surgery used for gerd, in my case 10 years ago i had a nissan wrap for the gerd,so now i can only have the ds, once you have a ds you wont have the option to have a fundus wrap as that is the part they toss out. it not there to wrap around the esophagas to close it off when the stomach contracts to push out the pylori, at the bottom, i would think that you would want the hiatal fixed perhap during the ds. it sits right on top of the stomach. the wrap proceedure is a same day surgery now, but that would require that part of your stomach to stay, and the ds part on the bottom of it. thus so far i have found only two ds doctors familar, gagner and herron. i know because they did a coworker, made me a standout in the patients that day.
