He specializes in body contouring for massive weight loss patients (which is what you want - or at minimum, a general plastic surgeon who does a BOATLOAD of these surgeries, since it's supposedly a complicated subspeciality). Some people in the NYC area also travel to DC to see Dr. Joseph Michaels - who's also a specialist in that area (massive weight loss patients)

Peter Fisher in San Antonio specializes in body contouring for massive weight loss patients (and you do want someone who specializes in this - or at minimum, someone who does a boatload of these surgeries). I've heard a ton of positive comments about Dr. Fisher's work.

I've lost ~200 lbs since my surgery back in 2004. I never did the plastic surgery due to cost.
I'm looking at having it done now.
Does anyone have any good surgery recommendations for the Austin area?
I know that Dr. Lomonaco In Houston tends to be the top choice but I'd rather not have to travel so far if an experienced surgeon with good results is in this area.

I don't have any experience with your surgery.
If you haven't had any success with your plastic surgeon, try & get an appointment with a gynaecologist.

IN THE END WE ONLY REGRET CHANCES WE DIDN'T TAKE. 
Has anyone experienced spreading of their labia after a medial thigh lift? I am 5 weeks post-op of an extended medial thigh lift (March 1, 2019). I think my surgeon didn't place my groin incisions high enough in the crease and it is pulling my labia apart. I asked her at my visit last week and she said it'll improve with time. But on another forum (RealSelf) another plastic surgeon said with tension and gravity it'll only get worse.
I would appreciate any input.
